Job Summary:

We are seeking a highly skilled and compassionate Licensed Clinical Social Worker LCSW to join our dynamic team at Sweetwater Care. As a key member of our care team, you will provide evidence-based therapy services to our patients and clients, working collaboratively with other professionals to ensure comprehensive care.

Responsibilities:

• Provide individual and group therapy sessions to patients and clients, using a variety of evidence-based modalities

• Develop and implement individualized treatment plans, incorporating patient goals and outcomes

• Collaborate with multidisciplinary treatment teams to ensure seamless care coordination

• Conduct assessments, evaluations, and treatment planning for patients and clients

• Maintain accurate and timely records of patient progress, consultations, and treatment plans

• Participate in case consultations and peer review to ensure best practices and quality care

Requirements:

• Master's or Doctoral degree in Social Work (MSW or DSW) from an accredited program

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Massachusetts
