New York Community Care is seeking a compassionate and dedicated Social Worker/Case Manager to join our team as an Adult Case Manager, providing support, advocacy, and coordination of services for adult clients.

Responsibilities include:

• Coordinate services and resources for adult clients

• Assess client needs and develop individualized service plans

• Advocate for clients and connect them with community resources

• Maintain accurate case records and documentation

• Collaborate with families, healthcare providers, and community organizations

• Provide culturally sensitive support to Russian-speaking clients

Qualifications:

• Fluent in Russian and English

• Bachelor’s or Master’s degree in Social Work, Human Services, Psychology, or related field preferred

• Experience in case management, social services, healthcare, or adult services preferred

• Excellent communication and organizational skills

• Compassionate, dependable, and client-focused
