MDS Coordinator – RN

The Mariemont Care Center is looking for an experienced MDS Coordinator to join our nursing team.

Responsibilities

• Coordinate and complete MDS assessments accurately and on time

• Manage the Resident Assessment Instrument (RAI) process

• Develop and update individualized care plans

• Coordinate with nursing, therapy, dietary, social services, and other departments

• Participate in interdisciplinary care plan meetings

• Review clinical documentation for accuracy and compliance

• Monitor MDS schedules and required deadlines

• Ensure documentation supports resident care and reimbursement

• Maintain compliance with CMS, Medicare, Medicaid, and state regulations

Qualifications

• Current, active RN license in Ohio

• Previous MDS experience required

• Long-term care/skilled nursing experience preferred

• Knowledge of MDS 3.0, RAI, and care planning

• Strong attention to detail and organization

• Ability to work independently and meet deadlines

• Strong communication and teamwork skills
