Speech Language Pathologist (SLP)

We are seeking a compassionate and dedicated Speech Language Pathologist (SLP) to join our rehabilitation team at Sunrise Point Health and Rehab, a skilled nursing and long-term care facility in Rockledge, FL. As a Speech Language Pathologist at our facility, you will have the opportunity to make a meaningful impact on the lives of our residents.

About the Role:

As a Speech Language Pathologist at Sunrise Point Health and Rehab, your primary responsibilities will include:

• Evaluating and treating residents with speech, language, communication, cognitive-communication, and swallowing disorders

• Developing and implementing individualized treatment plans based on residents' needs and physician orders

• Providing dysphagia assessments and interventions to promote safe swallowing and appropriate nutrition

• Collaborating with nursing, dietary, physicians, and other rehabilitation professionals to coordinate resident care

• Educating residents, families, and caregivers regarding communication strategies, swallowing precautions, and treatment goals

• Completing accurate and timely evaluations, progress notes, treatment documentation, and discharge summaries

What We're Looking For:

• Master's degree in Speech-Language Pathology from an accredited program

• Current, active Florida Speech-Language Pathologist license

• Current CPR certification, if required by facility policy

• Ability to successfully complete all required background screenings

• Strong communication, organizational, and clinical documentation skills

• Commitment to providing compassionate, resident-centered care

Preferred Qualifications:

• Previous experience in a skilled nursing facility or long-term care setting

• Experience treating dysphagia, cognitive-communication disorders, and neurological conditions

• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred

• Familiarity with Medicare documentation requirements and interdisciplinary care planning
