About the Role

As a Licensed Practical Nurse (LPN), you will deliver comprehensive nursing care by performing physical examinations, vital signs assessments, and patient evaluations to inform treatment plans. You will also administer medications and injections accurately, assist with diagnostic procedures, and manage complex patient needs.

Responsibilities

• Deliver comprehensive nursing care by performing physical examinations, vital signs assessments, and patient evaluations to inform treatment plans
• Administer medications and injections accurately, including IV infusions and catheterizations, following established protocols and safety standards
• Assist with diagnostic procedures such as specimen collection, processing, and medical imaging coordination to support accurate diagnoses
• Manage complex patient needs including acute pain management, dementia care, hospice support, and infectious disease precautions
• Maintain detailed medical documentation within electronic health record (EHR) systems like PCC; ensuring compliance with HIPAA and long-term care regulations
• Collaborate with multidisciplinary teams to develop discharge planning strategies, case management plans, and client communication efforts for optimal patient outcomes
• Provide education on medication administration, physical examinations, first aid techniques, and caregiver communication to patients and their families

Requirements

• Valid state licensure as a Licensed Practical Nurse (LPN) with current certification in CPR/BLS
• Proven experience working with diverse patient populations such as geriatrics, pediatrics, or individuals with disabilities
• Demonstrated proficiency in EMR/EHR systems like PCC for medical documentation and patient records management
• Knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/ICD-10), and infection control practices
• Strong communication skills for effective interaction with patients, families, caregivers, and healthcare teams; experience working with individuals with developmental disabilities or dementia is a plus
