Current Opportunity
We are currently seeking a PRN Registered Nurse (RN), preferably, or Licensed Practical Nurse (LPN) to provide intermittent skilled nursing visits in the Elkhart/Goshen area, with the ability to assist with visits in South Bend when needed.
Shifts and visits are available now!
An RN is preferred, particularly for patient admissions and start-of-care visits. However, qualified LPN candidates are encouraged to apply.
Current Scheduling Needs
The initial schedule is approximately 2 days per week, with additional PRN opportunities available as patient needs grow.
Nursing visits are compensated on a per-visit basis, with the current schedule including:
One nursing visit on Fridays
One nursing visit every other Sunday
Additional PRN visits as patient needs increase
Pay: $40–$50 per nursing visit through Firefly Home Health
There may also be opportunities for care management work through First Call Care Management, paying approximately $45 per hour.
Nursing Responsibilities
Provide intermittent skilled nursing visits in patients’ homes
Complete nursing assessments and monitor changes in patient condition
Follow individualized plans of care and physician orders
Provide education and support to patients and their families
Maintain accurate and timely clinical documentation
Communicate with physicians, care coordinators, and other members of the care team
Assist with admissions and start-of-care visits when qualified and needed
Travel throughout the Elkhart/Goshen area and occasionally to South Bend for scheduled visits
Qualifications
Active Indiana RN or LPN license in good standing
RN preferred, particularly for admission visits
Previous home health, skilled nursing, or geriatric care experience preferred
Strong assessment, communication, and documentation skills
Dependable, compassionate, and professional
Reliable transportation and a valid driver’s license
Ability to pass all required background screenings
