Director of Rehab Services

Lamar Estates, a small-town Skilled Nursing Facility in Lamar, CO, is seeking a motivated and experienced Director of Rehab Services to lead our rehabilitation program and help provide high-quality care to our residents.

About the Role

The Director of Rehab Services will oversee the daily operations of our rehabilitation program, support our therapy team, and work closely with clinical staff to ensure positive outcomes for our residents.

Responsibilities

• Oversee daily rehabilitation operations, including scheduling, staffing, and budgeting

• Provide clinical direction and support to physical, occupational, and speech therapy staff

• Develop and maintain policies and procedures in compliance with state and federal regulations

• Coordinate with physicians, hospitals, and other healthcare providers to support continuity of care

• Participate in resident care planning and ensure therapy goals are implemented

• Monitor quality and identify opportunities to improve rehabilitation outcomes

Requirements

• At least 2 years of experience as a practicing therapist

• Current licensure as a PTA, COTA, PT, OT, or SLP
