Job Summary:

Laurellwood Post-Acute and Rehab is seeking a compassionate and skilled LPN to join our dynamic healthcare team. As a key member of our care team, you will play a vital role in providing exceptional patient care, promoting independence, and empowering our patients to reach their full potential. Responsibilities:

• Provide direct patient care, including administering medications, wound care, and vital sign monitoring

• Monitor patient progress, identify potential issues, and report concerns to the nursing team

• Assist with patient admissions, discharges, and transfers

• Maintain accurate and timely records of patient care, treatments, and observations

Requirements:

• LPN certification or diploma

• 0 years of experience (entry-level candidate welcome)

• Strong communication and interpersonal skills

• Ability to work effectively in a fast-paced environment
