Registered Nurse (RN) Home Health

Vertus HealthCare is seeking experienced Registered Nurses (RNs) to provide quality skilled nursing care to patients in the comfort of their homes throughout Palm Beach County.

Responsibilities:

• Provide skilled nursing visits based on the patient’s plan of care.

• Complete OASIS, SOC, ROC, Recertification, and Discharge visits as assigned.

• Assess patient needs and coordinate care with the clinical team.

• Complete timely and accurate clinical documentation.

• Communicate with physicians, patients, families, and the healthcare team.

Qualifications:

• Current Florida RN license.

• Home Health experience preferred.

• OASIS experience preferred.

• Strong clinical assessment and documentation skills.

• Reliable transportation.
