LPN Clinical Intake & Scheduling Coordinator Medicare Home Health

Vertus HealthCare is seeking a highly organized and detail-oriented Clinical Intake Coordinator with Medicare Home Health experience to join our growing team.

The ideal candidate understands the Home Health referral and intake process and can accurately review patient information, insurance, physician orders, clinical documentation, and Medicare requirements while coordinating timely admissions.

Responsibilities:

• Process and coordinate Medicare Home Health referrals from hospitals, physicians, SNFs, and other referral sources.

• Review patient demographics, insurance, orders, clinical information, and required documentation for accuracy and completeness.

• Verify eligibility and identify missing information before admission.

• Coordinate SOC scheduling with the clinical team.

• Communicate with referral sources, patients, families, and clinicians.

• Follow up on pending referrals and documentation.

• Maintain accurate intake records and ensure timely processing.

Qualifications:

• Medicare Home Health intake experience strongly preferred.

• Strong attention to detail and accuracy.

• Knowledge of Medicare Home Health referral and admission requirements.

• Experience reviewing clinical documentation and insurance information.

• Excellent communication and organizational skills.

• Home Health EMR experience preferred.

• Ability to prioritize multiple referrals in a fast-paced environment.
