Responsibilities:

• Assist clients with daily living activities, such as bathing, dressing, and grooming, in a safe and respectful manner

• Provide personal care, including toileting, transferring, and ambulation assistance as needed

• Monitor and report changes in clients' condition to licensed nursing staff

• Assist with medication reminder.

• Develop and maintain positive relationships with clients and their families, fostering a supportive and inclusive environment

• Collaborate with interdisciplinary teams to develop and implement individualized care plans, promoting effective communication and coordination of care

Requirements:

• 1 Year experience

• Drive's License

• High school diploma or equivalent required

• Ability to lift 25 pounds or more

• Strong communication and interpersonal skills
