Responsibilities:

• Provide direct patient care to clients in their homes, including assistance with activities of daily living (ADLs) such as bathing, dressing, and grooming

• Monitor and report changes in client condition

• Medication reminder

• Maintain accurate and detailed records of client care and services provided

• Demonstrate a commitment to excellence in patient care, professionalism, and customer service

Requirements:

• High school diploma or equivalent required

• Ability to work in a fast-paced, dynamic environment

• Excellent communication and interpersonal skills

• Reliable transportation and a valid driver's license
