As an OT at Ready2Nurse, you will partner with our senior patients to restore function and help them live at their personal best. You'll evaluate patients in their homes, develop OT plans of care, and deliver evidence-based treatment across prevention, assessment, intervention and rehabilitation.
This is a per diem position: you're paid per visit and set your availability, with visits scheduled around patient needs. We serve Santa Clara County and the broader Bay Area, and the role requires driving to patients' homes.

Required
Credentials: Current California Occupational Therapist license in good standing; graduate degree or higher in occupational therapy; current BLS/CPR.
Experience: Proven work experience as an occupational therapist with current knowledge of evidence-based treatment practices.
Clinical Expertise: Ability to evaluate, plan and treat common geriatric conditions such as stroke recovery, fall risk, Parkinson's, dementia/Alzheimer's, orthopedic and post-surgical recovery, and deconditioning.
Transportation: Valid driver's license and current auto insurance; ability to travel to patient homes throughout Santa Clara County and the Bay Area.
Onboarding: Eligibility to work in the United States (I-9), routine background check, health questionnaire and physical exam including recent TB screening, Tdap, MMR and Hepatitis B surface antigen/antibody.
Communication: Excellent interpersonal skills and the ability to work with patients and families of many different personalities.
Preferred
Home Health Experience: Prior home health OT experience, including OASIS-related documentation and Medicare home health requirements.
EHR: Working knowledge of WellSky (Kinnser) or similar electronic clinical documentation.
Professional Growth: Drive to continually learn and grow, with consistently professional behavior.
Evaluation & Planning: Complete OT evaluations and re-evaluations, identify patient goals, and develop individualized OT plans of care.
Treatment: Provide personalized, evidence-based interventions that improve strength, flexibility, coordination and activities of daily living (ADLs).
Independence & Safety: Offer cost-effective treatments and home safety recommendations that improve function, reduce fall risk and support independence, reducing reliance on medications and surgery where possible.
Wellness Programs: Create wellness-oriented home programs tailored to each patient's needs, and educate patients and caregivers.
Patient Engagement: Motivate patients during treatment to help them function optimally.
Care Coordination: Consult with nurses, physical and speech therapists, physicians and families; participate in care conferences as needed.
Progress Monitoring: Evaluate the effects of treatment, monitor progress and communicate changes to the care team.
Documentation: Document all patient care services timely and accurately in Ready2Nurse's EMR, including discharge summaries.
This position is per diem with no benefits. You are paid a fixed rate per visit type, plus mileage reimbursement in accordance with federal and state law.
Routine Visit / OT Discharge: $140 per visit
OT Evaluation / Re-evaluation: $155 per visit
Payment: Issued monthly after invoice is submitted and all required documentation is complete.
Schedule: Flexible, as-needed visits based on patient census and your availability.
