As a CHHA at Ready2Nurse, you will be the team member who spends the most hands-on time with our patients. You'll provide personal care, observe and report changes in condition, and document the care you deliver, helping our patients stay safe, comfortable and independent at home.
You'll work under the supervision of a registered nurse or therapist and be assigned to patients by an RN in a way that promotes quality, continuity and safety of care. This is a per diem position: you're paid per visit or per hour and set your availability, with visits scheduled around patient needs.

Required
Training & Competency: Successful completion of one of the following:
A training and competency evaluation program that meets 42 CFR §484.80(b) and (c); or
A competency evaluation that meets 42 CFR §484.80(c); or
A state-approved nurse aide training and competency evaluation meeting §483.151–§483.154, with current good standing on the state nurse aide registry; or
The requirements of a state licensure program that meets §484.80(b) and (c).
Credentials: Current CPR certification, maintained throughout employment. At least 18 years of age.
Documentation: Ability to read and follow written instructions and accurately document care given.
Independence: Self-directing, with the ability to work with little direct supervision in patients' homes.
Transportation: Licensed driver with a vehicle in good working order, insured in accordance with Organization requirements.
Personal Qualities: Empathy for the needs of the ill, injured, frail and impaired. Demonstrates tact, patience and good personal hygiene.
Preferred
Registry: Proof of registration on the Home Care Aide Registry through the Department of Social Services, Home Care Services Bureau.
Geriatric Experience: Experience caring for seniors with common conditions such as dementia/Alzheimer's, Parkinson's, stroke recovery and fall risk.
Skills Competencies: Demonstrated competency in vital signs, range of motion and other simple procedures delegated by nursing or therapy.
EHR: Comfort with electronic documentation on a mobile device; working knowledge of WellSky (Kinnser) a plus.
Personal Care: Provide hands-on care including baths, back rubs, oral hygiene, shampoos, dressing and undressing, toileting assistance, changing bed linen, and skin care to prevent breakdown.
Nutrition: Plan and prepare nutritious meals and assist with feeding when necessary.
Vital Signs: Take and record oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with completed/demonstrated skills competency).
Mobility & Therapy Support: Assist with ambulation and exercise according to the plan of care. Perform range of motion and other simple procedures as an extension of therapy services, as ordered (with skills competency).
Nursing Support: Perform simple procedures as an extension of nursing services, as ordered (with skills competency). Assist patients in the self-administration of medication.
Home Environment & Safety: Keep the patient's living area clean and orderly and do laundry as appropriate. Meet patient safety needs and use equipment (foot stools, side rails, etc.) safely and properly.
Observation & Reporting: Observe patients and report their condition and any significant changes to the assigned nurse.
Documentation & Conduct: Adhere to the Organization's documentation and care procedures and standards of personal and professional conduct.

The CHHA will not function in any manner viewed as the practice of nursing under the state's Nurse Practice Act. Specifically, the CHHA will not administer medications, take physician's orders, or perform procedures requiring the training, knowledge and skill of a nurse, such as sterile techniques.
Employment Type: Per diem. No benefits are offered with this position.
Compensation:
Medicare shower visit: 75 USD per visit
HHA visits: 35 USD per hour
Schedule: Flexible, as-needed visits based on patient census and your availability. Ability to travel between patient homes in the San Jose area is required.
