About the Role:

Responsibilities:

• Provide direct care to residents, including assistance with daily living activities, meals, and mobility

• Monitor residents' conditions and report any changes or concerns to the nurse manager or supervisor

• Collaborate with the interdisciplinary team to develop and implement individualized care plans

• Maintain accurate and timely records of resident care and services

• Participate in quality improvement initiatives and maintain a commitment to our center's mission and values
