About the Role:

We are seeking a compassionate and organized individual to serve as our Hospice Community Liaison. In this Hospice Community Liaison position, you will be responsible for developing and maintaining relationships with community partners, healthcare providers, and other stakeholders to ensure seamless coordination of care. Your strong communication and interpersonal skills will be essential in building trust and fostering collaboration with our partners to promote patient-centered care.

Key Responsibilities:

• Develop and maintain relationships with community partners, healthcare providers, and other stakeholders to ensure effective coordination of care

• Identify and address community needs, and develop strategies to address these needs

• Collaborate with healthcare providers and other stakeholders to promote patient-centered care

• Provide education and support to patients, families, and caregivers

• Participate in case conferences and other meetings to ensure continuity of care

• Maintain accurate records and documentation of patient interactions and care coordination activities
