Position Overview

Homeside Rehab is expanding our therapy team in Piscataway, New Jersey and is currently seeking a licensed Physical Therapist to provide one-on-one rehabilitation services in patients’ homes.

This opportunity is ideal for a PT who values independence, flexible scheduling, and the ability to build meaningful relationships with patients and families. You will work closely with our clinical and administrative teams while managing a caseload within your assigned service area.

What You’ll Do

• Complete comprehensive physical therapy evaluations in the home setting

• Develop personalized plans of care based on patient needs, functional limitations, and rehabilitation goals

• Provide skilled physical therapy interventions to improve mobility, strength, balance, coordination, and overall function

• Monitor patient progress and adjust treatment recommendations when appropriate

• Educate patients and caregivers on exercises, mobility techniques, safety strategies, and home programs

• Complete electronic documentation accurately and within required timeframes

• Communicate with physicians, caregivers, office staff, and other members of the care team as needed

• Maintain compliance with company policies, professional standards, and applicable state requirements

Qualifications

• Active New Jersey Physical Therapist license required

• Graduate of an accredited Physical Therapy program

• Strong communication, organization, and clinical decision-making skills

• Comfortable working independently in a home-based setting

• Ability to complete electronic documentation

• Valid driver's license and reliable transportation

• Prior experience in home health, outpatient rehabilitation, skilled nursing, or a related clinical setting preferred

• At least 1 year of clinical experience preferred

Position Details

Location: Piscataway, NJ 08854
Pay: $65 - $80 per hour
Employment Options: Full-Time, Part-Time, or Per Diem

Scheduling may include:

• Flexible daytime and evening availability

• Monday through Friday opportunities

• Weekend availability based on patient needs

• Ability to coordinate visits around your existing schedule
