Job Summary:

We are seeking a talented and dedicated Licensed Dietitian to join our nutrition team! As a Licensed Dietitian, you will be responsible for developing and implementing individualized nutrition plans for our patients and residents. This is a full-time position that offers the opportunity to work in a dynamic and supportive environment, with a focus on patient-centered care.

Responsibilities:

• Conduct nutrition assessments and develop individualized nutrition plans for patients and residents

• Collaborate with interdisciplinary teams to provide comprehensive care and support

• Monitor patient progress and adjust nutrition plans as needed

• Educate patients and their families on nutrition and wellness principles

• Participate in meetings and contribute to quality improvement initiatives

• Maintain accurate and timely records of patient care and treatment

Requirements:

• Must be a licensed dietitian with a Bachelor's or Master's degree in Nutrition or a related field

• Minimum of 0 years of experience in a healthcare setting

• Strong communication and interpersonal skills

• Ability to work effectively in a fast-paced environment

• Familiarity with electronic medical records and nutrition software
