• Assess and treat patients with communication and swallowing disorders in a home care setting

• Develop and implement individualized treatment plans to achieve patient goals and improve quality of life

• Collaborate with patients, families, and healthcare providers to ensure seamless care coordination

• Conduct frequent assessments and progress monitoring to adjust treatment plans as needed

• Maintain accurate and detailed records of patient treatments and outcomes

• Participate in multidisciplinary team meetings to share knowledge and best practices

• Stay up-to-date with industry developments and advancements in speech therapy techniques and technology

Requirements:

• Speech Therapist degree and licensure in the state of New York

• 1-2 years of experience (entry-level candidates encouraged to apply)

• Ability to work independently in a home care setting

• Strong communication and interpersonal skills
