This full-time, on-site role supports residents, families, and the care team through discharge planning, psychosocial support, and coordination of services.

Responsibilities

• Lead and coordinate discharge planning to ensure safe and timely transitions

• Provide psychosocial support to residents and families

• Collaborate with nursing, therapy, and leadership on care plans and care conferences

• Support admissions, transitions, and resident assessments

• Connect residents and families with community resources and services

• Ensure compliance with state and federal regulations, including Centers for Medicare & Medicaid Services guidelines

• Maintain accurate, timely, and compliant documentation

Qualifications

• Bachelor’s degree in Social Work or a related field required

• Experience in skilled nursing, rehabilitation, or long-term care preferred

• Strong communication, organization, and problem-solving skills

• Ability to manage multiple priorities in a fast-paced environment

• Licensure not required

• Candidates with LBSW or LMSW are encouraged to apply
