Social Worker – Nursing Home / Long-Term Care (LTC)

Join a modern, beautifully maintained skilled nursing and rehabilitation center in Far Rockaway as our next Social Worker. You'll be part of a five-person social work team in a warm, family-like building with a highly regarded rehab department. This is a staff Social Worker role, not a director position.

This Social Worker position requires experience in one of four settings: nursing home, assisted living (AL), memory care, or home care. Social Workers coming from assisted living, memory care, or home care will be trained on the skilled nursing facility (SNF) side.

Social Worker Duties

• Complete psychosocial assessments and care plans for residents

• Plan safe discharges for short-term rehab residents

• Serve as an advocate and point of contact for residents and families

• Participate in interdisciplinary care plan meetings

• Coordinate community resources and post-discharge services

Social Worker Compensation and Schedule

• Pay: $55,000 – $65,000 per year

• Full-time, day shift

• Location: Far Rockaway, NY 11691

Social Worker Qualifications

• Required: Social Worker experience in a nursing home, assisted living (AL), memory care, or home care setting

• Applicants without experience in one of these four settings will not be considered. Hospital, clinic, school, child welfare, and behavioral health experience alone does not qualify.

• Your resume must clearly show the setting for each Social Worker role you've held.
