Responsibilities:

• Provide direct patient care to residents, including administering medications, performing wound care, and assisting with daily living activities

• Monitor and report changes in resident condition to licensed nurses and physicians

• Develop and implement individualized care plans in collaboration with the healthcare team

• Participate in interdisciplinary team meetings to discuss resident progress and plan for ongoing care

• Maintain accurate and detailed records of patient care and medication administration

• Collaborate with therapy staff to ensure comprehensive rehabilitation services

• Maintain a safe and clean environment for residents, staff, and visitors

Requirements:

• Current LPN licensure in the state of Missouri

• High school diploma or equivalent required; associate's or bachelor's degree preferred

• 0 years of experience in a skilled nursing or rehabilitation setting; recent graduates welcome to apply

• Ability to work varying shifts, including evenings, weekends, and holidays
