Responsibilities:

• Provide direct care to residents, including assistance with activities of daily living (ADLs) such as bathing, dressing, and toileting

• Assist with medication administration, vital sign monitoring, and wound care as directed by licensed nursing staff

• Maintain accurate and thorough documentation of resident care and services

• Develop and implement individualized care plans, ensuring resident-centered care and promoting independence

• Participate in interdisciplinary team meetings to ensure comprehensive care and address resident needs

• Demonstrate a commitment to resident safety and falls prevention, promoting a safe and supportive environment
