Director of Social Services

Alliance Health at Coleman is a not-for-profit 45-bed skilled nursing center in Northborough, MA. We are looking for full-time Social Worker.

Responsible to provide social work services and support to residents and their families. Responsible for the completion of required documentation of psychosocial visits and assessments.

Education required: Bachelor’s degree in Social Work or related field

Professional licensure required: Massachusetts LCSW, or LICSW.

Long Term Care Social Worker must communicate effectively with residents, families, visitors, and interdisciplinary team to support the emotional, mental, and clinical well-being of residents. Familiar with Medicare and Medicaid regulations and procedures. Plans and facilitates the weekly Care Plan meetings. Completes the PASARR requirements in accordance with regulations.

Experience in long term care social work preferred, as well as knowledge of Medicare and Medicaid
