Registered Nurse (RN) – Long-Term Care

Fairport Rehab & Nursing Center is seeking a compassionate Registered Nurse (RN) to provide high-quality, resident-centered care in our long-term care setting. The RN will collaborate with residents, families, providers, and the interdisciplinary team to promote safety, dignity, comfort, and positive outcomes.

Responsibilities

• Provide direct nursing care according to care plans, physician orders, and facility policies.

• Administer medications and treatments safely and accurately.

• Assess residents and monitor changes in condition.

• Develop and evaluate individualized care plans.

• Communicate with residents, families, providers, and the healthcare team.

• Maintain accurate clinical documentation and medical records.

• Provide education and support to residents and families.

• Follow infection prevention, safety, and quality standards.

• Participate in quality improvement initiatives.

Qualifications

• Current, unrestricted New York State RN license.

• Long-term care/skilled nursing experience preferred.

• Strong clinical, communication, and organizational skills.

• Ability to work effectively with an interdisciplinary team.

• Compassionate, professional, and resident-focused approach.

Compensation & Schedule

• $40.00–$42.00/hour, based on certification and post-license experience.

• Shift differentials: +$1.50/hour for weekday evening/night shifts; +10%/hour for weekends.

• Shifts: Days (7 AM–3 PM), Evenings (3 PM–11 PM), Nights (11 PM–7 AM).

• Full-Time: 37.5 hours/week | Part-Time: 22.5 hours/week.

• Every other weekend required.

• $4,000 sign-on bonus for Full-Time RNs.
