Responsibilities:

• Provide direct patient care to residents, including administering medications, wound care, and other treatments as prescribed by physicians

• Develop and implement individualized care plans in collaboration with the multidisciplinary team

• Conduct routine assessments and monitor patient progress to ensure optimal outcomes

• Collaborate with dietary and activities staff to develop and implement patient-centered care plans

• Maintain accurate and up-to-date patient records and reports

• Participate in quality improvement initiatives to optimize patient care and outcomes

Requirements:

• Must be licensed as a Licensed Practical Nurse (LPN) in the state of Tennessee

• Minimum of 0 years of experience in a long-term care or acute care setting

• High school diploma or equivalent required
