Occupational Therapist OT

Wesley Gardens Nursing and Rehabilitation Center is looking for a dynamic Occupational Therapist for one of our skilled nursing facilities! We are dedicated to improving the lives of our residents and would love to have you join our team of passionate therapists who spend every day making a difference in the lives of so many!

DUTIES AND RESPONSIBILITIES:

• Develop and implement the Occupational Therapy plan of care for facility residents and short-term patients.

• Assist as needed with patient screenings for the need for Occupational Therapy.

• Communicate progress and any barriers toward OT goals with interdisciplinary team members.

• Complete timely OT documentation in compliance with regulatory, licensing, and payor requirements.

• Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.

• Provide and document patient, family, and caregiver training regarding each OT patient’s specific plan of care and recommendations for follow up.

• Perform additional duties as required.

Education and Experience Requirements:

• NYS Licensed Occupational Therapist

• One year of occupational therapy experience in a geriatric setting preferred but not required.
