Amaryllis Nursing and Rehab Center, a leading provider of healthcare services, is seeking a compassionate and skilled Licensed Practical Nurse to join our team of dedicated healthcare professionals in Smethport, PA. As a Staff LPN, you will play a vital role in providing high-quality patient care and contributing to the overall success of our organization.

Responsibilities:

• Evaluate and identify changes in residents' conditions and initiate appropriate steps

• Provide direct patient care, including administering medications, conducting vital signs, and performing treatments

• Demonstrate excellent interpersonal and communication skills to ensure effective collaboration with the healthcare team

• Participate in care planning and development of individualized care plans

• Maintain accurate and detailed records of patient care and treatment

• Collaborate with other healthcare professionals to ensure seamless transitions of care

• Provide emotional support and empathy to patients and families

• Maintain a clean, safe, and organized environment

Qualifications:

• Licensed as an LPN

• Previous experience in a long-term care facility

• Excellent interpersonal and communication skills

• Patient and emotionally mature individual

• Ability to make independent decisions with good judgment and collaborate as a team player
