What You'll Do

• Conduct psychosocial assessments and develop individualized care plans

• Support residents and families with discharge planning, adjustment to facility life, and advocacy

• Coordinate with nursing, activities, and administration to ensure residents' emotional and social needs are met

• Maintain accurate, timely documentation in compliance with state and federal regulations

• Serve as a liaison between residents, families, and community resources

What We're Looking For

• Master's degree in Social Work (MSW)

• Active state licensure (LSW/LCSW or equivalent) in good standing

• Prior experience in long-term care, skilled nursing, or a healthcare setting preferred

• Strong communication skills and a genuine passion for resident-centered care

• Ability to pass required background check, TB screening, and other pre-employment clearances
