About the Role:

As a Dietitian at Morristown Post Acute, you will be responsible for:

• Conducting dietary assessments and developing individualized meal plans for patients with various medical conditions

• Collaborating with healthcare teams to develop and implement dietary interventions to improve patient outcomes

• Monitoring patient progress and adjusting dietary plans as needed

• Providing education and support to patients, families, and caregivers on healthy eating habits and disease management

• Participating in quality improvement initiatives to ensure the highest level of care and patient satisfaction

Responsibilities:

• Conduct nutritional assessments and develop individualized meal plans for patients with cardiac, orthopedic, renal, and pulmonary issues

• Collaborate with multidisciplinary teams to develop and implement comprehensive care plans

• Monitor patient progress and adjust dietary plans as needed to achieve optimal outcomes

• Educate patients, families, and caregivers on healthy eating habits and disease management

• Participate in quality improvement initiatives to ensure the highest level of care and patient satisfaction
