Lancaster Nursing & Rehabilitation Center is excited to welcome an experienced and compassionate LPN Wound Care Coordinator to our nursing team!
This is a full-time, hourly clinical position focused on coordinating and supporting the facility’s wound care program. The Wound Care Coordinator works closely with the Director of Nursing, nursing staff, physicians, and the interdisciplinary team to promote skin integrity, support wound healing, and help ensure high-quality resident care.
The typical schedule for this position is 8:00 AM–4:00 PM, with hours subject to change based on facility needs and the direction of the Director of Nursing.
Coordinate and support the facility’s day-to-day wound care program in accordance with nursing standards, facility policies, and applicable regulations.
Complete accurate and timely wound documentation, including nursing notes and required wound tracking.
Perform and document skin and body assessments on new admissions as required by facility policy.
Assess residents at increased risk for skin breakdown and help implement appropriate pressure-reduction and prevention interventions.
Perform wound treatments within the LPN scope of practice and help ensure ordered wound treatments are completed as scheduled.
Photograph pressure areas and wounds in accordance with facility protocol.
Monitor residents for changes in skin integrity and wound condition and promptly communicate concerns to the Director of Nursing or appropriate clinical team member.
Participate in weekly wound care meetings and Quality Assurance activities related to resident care.
Assist with staff education and in-service training related to wound care, skin integrity, infection control, and proper nursing practices.
Maintain compliance with infection-control, PPE, hand hygiene, isolation, and universal precaution practices.
Collaborate with nursing staff and other departments to help ensure residents receive appropriate and timely care.
Maintain accurate clinical documentation, reports, and records related to wound care.
Provide direct nursing care as necessary.
Participate in continuing education and maintain all required licenses and certifications.
Perform additional related nursing duties as assigned.
Graduate of an accredited school of nursing.
Current, active, and unencumbered Licensed Practical Nurse (LPN) license in Pennsylvania.
Previous long-term care experience preferred.
Wound care experience preferred.
Wound care certification or specialized wound care training is a plus.
Knowledge of nursing practices, procedures, and regulations governing long-term care.
Strong clinical assessment, documentation, communication, and organizational skills.
Ability to work independently and as part of an interdisciplinary team.
Ability to communicate effectively with residents, families, nursing staff, physicians, and other members of the care team.
