$70,000-$85,000 per year

At The Hamilton Assisted Living, we believe that resident wellness is achieved through compassionate clinical leadership, consistent care delivery, and strong interdisciplinary teamwork. Guided by our core values of Passion, Respect, and Excellence, the Wellness Coordinator oversees resident care services to ensure clinical quality, regulatory compliance, and a high standard of daily operations within the Resident Care Department.

Responsibilities for Wellness Coordinator (LPN):

• Oversee all areas of the Resident Care Department, including compliance, clinical planning, assessments, staffing, budgeting, and direct resident care.

• Ensure resident care needs are met and maintained in accordance with physician orders, state regulations, and company policies.

• Conduct and oversee resident assessments and ensure appropriate care plans are developed, implemented, and updated as needed.

• Provide clinical leadership, guidance, and support to nursing and care staff to ensure the delivery of high-quality care.

• Monitor staffing levels and shift schedules to ensure adequate coverage and efficient operations.

• Support regulatory compliance through ongoing audits, documentation review, and adherence to facility standards.

• Collaborate with interdisciplinary teams to ensure coordinated, resident-centered care.

• Promote a culture of accountability, teamwork, and continuous improvement within the department.

Qualifications for Wellness Coordinator (LPN):

• Graduate of an accredited nursing program with a current, active, and unencumbered LPN license in the state of Virginia.

• Minimum of 3 years of clinical experience in a long-term care setting required.

• Valid BLS Certification (CPR) required.

• Strong clinical assessment and critical thinking skills.

• Demonstrated ability to manage multiple priorities in a fast-paced healthcare environment.

• Experience in leadership, staffing coordination, or supervisory responsibilities preferred.
