Responsibilities:

• Coordinate with hospital staff to schedule and facilitate patient discharges and transfers

• Verify patient information and treatment plans to ensure accuracy and completeness

• Communicate with patients and their families to provide guidance and support throughout the transition process

• Collaborate with interdisciplinary teams to develop and implement individualized care plans

• Monitor and report on patient outcomes and satisfaction to inform quality improvement initiatives

Requirements:

• Hospital Liaison experience preferred
