Job Summary:

Carease Health - FL is seeking a reliable and experienced Medical Assistant or Certified Nursing Assistant (CNA) to join our team! As a Medical Assistant at Carease Health, you will be an integral part of our preventative healthcare program, focused on reducing hospitalizations and improving the lives of nursing home patients through remote monitoring, chronic care management, and psychiatric collaborative care.

Responsibilities:

• Take vital signs and perform related tasks as needed

• Provide exceptional patient care and support to our clinical team

• Work efficiently in a fast-paced environment, prioritizing tasks and managing multiple responsibilities

• Collaborate with healthcare professionals to promote proactive measures for enhanced physical and mental health

• Participate in the implementation of Carease Health's comprehensive preventative care program, including Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM)

Requirements:

• 2 years of experience as a Medical Assistant or Certified Nursing Assistant (CNA)

• Strong organizational skills and ability to work efficiently in a fast-paced environment

• Skilled in taking vital signs

• Ability to relocate before starting work

• Driver's License and Certified Medical Assistant (CMA) certification required

• High school diploma or equivalent required; Associate's degree preferred

Schedule:

• Monday to Friday shifts, no weekend work
