About the Role:

• Provide direct care to residents, including bathing, dressing, grooming, and feeding

• Assist with transfer, ambulation, and positioning residents

• Take vital signs and monitor resident condition

• Document resident care and maintain accurate records

• Collaborate with nursing staff to develop and implement individualized care plans

• Support residents with daily living activities, such as toileting, incontinence care, and medication management

• Maintain a clean and safe environment, adhering to infection control and safety protocols

Responsibilities:

• Provide exceptional patient care, prioritizing resident needs and preferences

• Develop strong relationships with residents, families, and healthcare team members

• Maintain accurate and thorough records, ensuring timely documentation and communication

• Participate in staff meetings, sharing ideas and contributing to quality improvement initiatives

• Assist with med passes, ensuring residents receive timely and accurate medication administration
