As a Medical Social Worker, you'll be more than a clinician. You'll be a trusted advocate, counselor, educator, and source of comfort for patients and families facing end-of-life challenges. You'll work alongside a dedicated interdisciplinary team to ensure each patient receives compassionate, individualized care that honors their wishes, values, and dignity.

What You'll Do

✅ Provide psychosocial assessments and develop individualized care plans

✅ Offer emotional support, counseling, and guidance to patients and families

✅ Help caregivers navigate stress, grief, loss, and end-of-life transitions

✅ Connect patients with community resources and support services

✅ Collaborate closely with nurses, physicians, chaplains, and other care team members

✅ Advocate for patient needs and coordinate services across care settings

✅ Maintain accurate documentation and participate in care planning meetings

✅ Travel throughout the service area to provide support where patients call home

What We're Looking For

Required:

• Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or a related field

• Minimum one year of healthcare experience

• Strong communication, counseling, and organizational skills

• Valid driver's license, reliable transportation, and current auto insurance

Preferred:

• Master of Social Work (MSW)

• Hospice, palliative care, home health, or healthcare social work experience

• Knowledge of hospice philosophy, grief support, and end-of-life care
