Job Summary:

As a Licensed Practical Nurse (LPN), you will be responsible for delivering high-quality care to our residents within our long-term care facility. You will work closely with our interdisciplinary team to develop and implement individualized care plans, ensuring our residents receive the best possible outcomes.

Responsibilities:

• Provide direct patient care, including administering medications, performing vital signs, and conducting wound care

• Collaborate with the healthcare team to develop and implement individualized care plans

• Perform routine clinical tasks, such as taking temperatures, blood pressures, and pulse oximetry

• Document patient information accurately and thoroughly

• Communicate effectively with patients, families, and healthcare team members

• Participate in quality improvement initiatives to optimize patient outcomes

• Work flexible schedules, including days, nights, and weekends

Requirements:

• Current LPN license in the State of Indiana

• Floor nurse experience preferred

• Ability to work 12-hour shifts

• Strong communication and interpersonal skills

• Ability to work effectively in a fast-paced environment

• Commitment to maintaining confidentiality and adhering to HIPAA regulations
