Care Manager (MSW Preferred) | Private Duty Home Care

We are seeking an experienced, organized, and compassionate Care Manager to join our growing private-pay home care team. This role is ideal for a social worker or aging services professional who excels at building relationships, conducting assessments, developing care plans, and helping families find the right support to remain safe and independent at home.

MSW candidates are preferred; qualified BSW candidates and other professionals with relevant experience in home care, aging services, care management, healthcare, or senior living will be considered.

As a Care Manager, you will serve as a trusted advisor to prospective and current clients, guiding families through the process of arranging private-duty home care services. From the initial inquiry through ongoing support, you will help families understand their options, assess care needs, coordinate services, and ensure an exceptional client experience.

Success in this role requires strong communication skills, excellent organization, sound judgment, and a passion for helping older adults age safely and comfortably in their homes. The ideal candidate is relationship-focused, proactive, detail-oriented, and comfortable working in a consultative environment that supports both client care and business growth.

Continuum Care Manager Responsibilities

• Respond to inquiries from prospective clients and families seeking private-duty home care services.

• Conduct in-home assessments and care consultations to evaluate client needs, home safety, and support requirements.

• Develop personalized care plans and recommend appropriate private-pay caregiving services.

• Educate clients and families about home care options, care schedules, and available support resources.

• Coordinate the initiation of services and facilitate successful caregiver-client introductions.

• Maintain ongoing communication with clients, families, and caregivers to ensure satisfaction and quality of care.

• Monitor changes in client conditions and adjust care plans as needed.

• Collaborate with hospitals, rehabilitation facilities, physicians, senior living communities, elder law professionals, and other referral sources.

• Build and maintain strong relationships within the community to promote Continuum's services and generate referrals.

• Document assessments, care plans, communications, and service updates accurately and timely.

• Manage a caseload of clients while ensuring exceptional customer service and follow-through.

• Participate in an on-call rotation to support clients, families, and caregivers after hours.

Qualifications for a Continuum Care Manager

• Master's Degree in Social Work (MSW) preferred; Bachelor's Degree in Social Work (BSW) considered.

• Experience in private-duty home care, care management, senior services, healthcare, social work, or a related field preferred.

• Certification in Care Management (CMC), Aging Life Care, or Gerontology is a plus.

• Strong assessment, relationship-building, and problem-solving skills.

• Exceptional communication and customer service abilities.

• Ability to navigate sensitive family dynamics and complex care situations with professionalism and empathy.

• Strong organizational skills and ability to manage multiple priorities.

• Proficiency with technology, documentation systems, and CRM software.

• Valid driver's license and reliable transportation for client visits and community networking activities.
