Aspire Senior living Warsaw is seeking a compassionate and skilled Physical Therapist (PT) to join our team at Aspire Senior Living Warsaw. As a Per Diem Physical Therapist, you will provide high-quality physical therapy services to our residents, promoting independence, comfort, and well-being.

Responsibilities:

• Evaluate and treat residents with a wide range of physical impairments, including those with chronic conditions, injuries, or surgical procedures
• Develop and implement personalized treatment plans to achieve residents' physical therapy goals
• Collaborate with other members of the interdisciplinary team to ensure comprehensive care
• Conduct physical therapy consultations with residents, families, and healthcare professionals
• Keep accurate and detailed records of treatment plans, progress notes, and evaluations
• Maintain a clean and organized treatment environment, adhering to safety standards and infection control protocols

Requirements:

• Currently licensed as a Physical Therapist (PT) in the state of Missouri
• Minimum of [insert minimum years of experience] years of experience in a physical therapy setting
