About the Role

We're seeking a dedicated and compassionate Registered Nurse (RN) to join our interdisciplinary care team at Walnut Creek Nursing and Rehab, a state-of-the-art skilled nursing facility in Erie, Pennsylvania. As an RN, you'll be responsible for providing high-quality, patient-centered care to our residents, promoting a positive and supportive environment, and collaborating with our multidisciplinary team to achieve optimal outcomes.

Responsibilities

• Provide direct patient care to residents, including assessing, planning, implementing, and evaluating individualized care plans

• Administer medications, treatments, and therapies as prescribed by physicians and consistent with facility policies

• Monitor and report changes in resident condition, including vital signs, pain levels, and other relevant health metrics

• Develop and maintain effective communication with residents, families, and healthcare team members to ensure a cohesive and supportive environment

• Participate in quality improvement initiatives and contribute to a culture of continuous learning and improvement

• Collaborate with other members of the healthcare team, including physicians, therapists, and support staff, to achieve optimal patient outcomes

Requirements

• Current licensure as a Registered Nurse (RN) in the state of Pennsylvania

• Bachelor's degree in Nursing or related field

• Minimum of 1 year of experience as a Registered Nurse (RN)
