Speech Language Pathologist (SLP)

Carlton Nursing Home is located in Brooklyn, NY. We're looking for an excellent Speech-Language Pathologist to join our therapy team and the Phoenixville family as a Per Diem employee!

Job Description

The Speech Language Pathologist is responsible for providing rehabilitative therapy to patients upon referral for communication, cognitive, and/or swallowing disorders following all regulatory and clinical practice requirements. This includes recovery, improvement in function, and, when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services.

This position reports to the Director of Rehabilitation Services.

Job Requirements

• Must be a graduate of an accredited program in Speech-Language Pathology

• Must be ASHA certified (CCCs)

• Must have current and active license with the state of New York

• Experience in long-term care or related setting preferred

• Experience holding active CCCs prior to CFY Supervision preferred

License:

• NY state licensure as SLP(Required)
