Job Summary:

We are seeking a dedicated and compassionate Physical Therapist (PT) to join our team at Fort Wayne Health Campus in Fort Wayne, Indiana. As a leading skilled nursing facility, we are committed to providing high-quality care and rehabilitation services to our residents. Our Physical Therapy Department is a vital part of our care team, working closely with residents, families, and other healthcare professionals to achieve optimal outcomes.

Responsibilities:

• Evaluate and treat residents with a variety of physical disabilities and conditions, including but not limited to, orthopedic, neurological, and cardiovascular disorders

• Develop and implement individualized treatment plans in collaboration with residents, families, and other healthcare professionals

• Conduct physical therapy assessments and progress reports to measure treatment effectiveness and make necessary adjustments

• Utilize a variety of therapy modalities, including but not limited to, exercise prescription, manual therapy, and modalities

• Collaborate with other healthcare professionals, including but not limited to, physicians, occupational therapists, and speech-language pathologists, to ensure comprehensive care

• Maintain accurate and thorough documentation of resident care and treatment outcomes
