iCare Health Network is looking for a Nurse Evaluator on a per diem basis.
Essential responsibilities:
Responsible for clinical evaluation and assessment and communication for patients in the acute care setting that are transitioning to iCare managed care centers
Cultivates trusting and synergistic relationships with hospitals and physicians who are discharging post-acute, skilled nursing patients and builds patient, family, staff and census relationships while playing an integral part in transitioning patients through the health care continuum.
Responsibilities to include ongoing management of complex case referrals as well as the potential to be assigned client hospitals in Connecticut.
Communicate clinical services that care centers provide to those entities that refer patients to Skilled Nursing Facilities, such as hospitals, doctors' offices, community centers and other health care entities.
Secure patient referrals from key healthcare organizations and review case management of prospective candidates for admission as residents to an iCare Short Term Rehab or Long-Term Care Center.
Orienting and familiarizing patients with iCare while inpatient in an acute care hospital setting, onboarding/enrolling patients in iCare clinical programs, identification of new patients, and accurate clinical evaluation of patient needs.
Patient assessment and interview (when indicated, at Liaison’s discretion), communicate patient healthcare needs prior to admission and coordinate admission with the team.
Assist patient and family as a resource in education as they transition through the health care continuum.
Qualifications:
Must be a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the State of Connecticut. RN level must have a minimum of five years of clinical experience, LPN must have a minimum of ten years of experience. RN preferred.
Hospital and/or Long Term Care experience of five years or more is preferred.
Relationship building, corporate, development or previous care transitions experience is highly desirable and an important element in securing each patient as they have many options to choose from.
Ability to multi-task and balance a multitude of competing priorities a must.
Must be knowledgeable of clinical programming in the skilled nursing and long term care environment.
Strong knowledge of local healthcare market. Ability to travel regularly within your community to multiple iCare locations.
Must be able to read, write, clearly speak and understand the English language. Must have working knowledge (experience preferred) with various software programs such as WORD, EXCEL, PUBLISHER.
Must have current drivers/ operator’s license.
Must possess the ability to deal tactfully with hospitals, senior programs, and referral sources.
Previous SNF Evaluator experience
Must possess excellent people and presentation skills and a professional appearance.
Must show ability to utilize effective time management skills as well as demonstrate effective verbal and written communication skills a must.
