The Nurse Cohort Manager for the Academic Health Center program plays a critical leadership role in building and sustaining a high-performing team of virtual nurses dedicated to the AHC partnership. This role serves as the primary operational link between PocketRN and Academic Health Center clinical teams, including Advanced Practice Providers (APPs) and Case Managers.
The Manager is responsible for ensuring the nurse cohort is proficient in AHC-specific tools, including EMR and communication tools. They will oversee patient care quality, nurse performance, and the reinforcement of PocketRN’s culture within the context of the Academic Health Center innovative virtual care delivery model.
Full-Time, Monday through Friday
9:00 AM -5:00 PM PST with rotating weekend coverage
Maintain ultimate accountability for the quality and consistency of care delivered to the AHC population, including acute post-discharge patients.
Oversee clinical workflows including patient rooming for APP visits, Post-Discharge Calls (PDC), and Remote Patient Monitoring (RPM).
Ensure all nursing activities and escalations strictly adhere to clinical protocols and evidence-based triage guidelines.
Support nurses in clinical decision-making and escalate patient care concerns appropriately
Lead with a “nurse-first” mindset and actively foster a culture of engagement, accountability, and collaboration
Serve as a visible example of PocketRN’s Vision, Mission, and Values
Champion change initiatives and promote a culture of continuous improvement
Create a psychologically safe, supportive environment where nurses feel heard, valued, and empowered
Share responsibility with other Nurse Cohort Manager(s) for day-to-day supervision of remote nursing teams providing 24/7 coverage for AHC, including daytime, after-hours, and weekend shifts.
Ensure appropriate staffing levels to meet patient care needs
Address attendance, performance, and conduct issues in a timely, fair, and consistent manner
Provide frequent, constructive feedback on nurse performance and documentation
Conduct performance evaluations and support professional growth plans
Partner with training specialists to deliver ongoing education and development opportunities
Assist with onboarding, mentoring, and integration of new nurses
Collect, manage, and act on nurse feedback throughout the employee lifecycle at PocketRN
Support day-to-day operations, including workflow coordination, scheduling needs and follow up on operational issues.
Monitor team workflows, identify barriers to efficient patient care and recommend process improvements.
Assist with quality audits, documentation review, and tracking of key program metrics.
Support implementation of new workflows, service lines, and program initiatives as the partnership grows.
Serve as day to day resource for nurses, helping resolve routine operational questions and escalating complex issues appropriately.
Advocate for nurses by surfacing insights and feedback related to clinical resources, workflows, technology, product experience, patient experience, and nurse experience
Collaborate cross-functionally to improve care delivery operations and nurse satisfaction
Serve as a liaison between the nursing workforce and leadership
Education: Associate’s Degree
Experience: Minimum 3 years of nursing leadership; experience in virtual care or telehealth preferred.
Licensure: Unencumbered RN license in California, with multistate or additional states preferred. Must maintain licensure.
Ability to work a variety of shifts and take leadership on-call as required
Technology: Mastery of or strong willingness to master the AHC technology stack.
Protocol Adherence: Demonstrated ability to implement and ensure strict adherence to standardized clinical protocols and triage decision-support tools.
Demonstrated leadership experience in a management or operations role
Strong understanding of how PocketRN fits strategically within the broader healthcare continuum
Proactive, solution-oriented mindset with strong critical-thinking skills
Comfortable wearing multiple hats and thriving in a fast-paced, evolving startup environment
