AMA LTC is growing, and we are seeking a dynamic, relationship-driven RN/LPN Clinical Liaison to join our team and help connect patients, families, referral partners, and healthcare providers with the skilled nursing services they need.
We are looking for an experienced Registered Nurse (RN) or Licensed Practical Nurse (LPN) who can combine strong clinical knowledge with exceptional communication and relationship-building skills.
The ideal candidate understands that a successful Clinical Liaison is more than a referral source — they are a trusted clinical resource, relationship builder, and advocate for patients and families throughout the transition of care.
In this role, you will work closely with hospitals, physicians, case managers, discharge planners, and other healthcare partners to identify appropriate patients for skilled nursing services, complete clinical reviews, and help coordinate smooth and timely admissions.

Lead and manage the facility’s admissions and marketing efforts
Develop and maintain strong relationships with hospitals, physicians, discharge planners, case managers, community organizations, and referral partners
Build and maintain a strong referral network to support census growth
Respond promptly and professionally to inquiries from prospective residents and families
Conduct tours and effectively communicate the services, programs, and benefits offered by the facility
Coordinate the admissions process from initial inquiry through admission
Monitor referral sources, admissions activity, and census trends
Develop creative strategies to increase visibility within the local community
Represent the facility at community events, healthcare networking opportunities, and outreach activities
Work closely with the Administrator and leadership team to develop and execute census-growth strategies
Maintain accurate records and ensure timely follow-up with prospective residents and referral sources
Previous experience in healthcare admissions, marketing, business development, or a related field
Long-term care, skilled nursing, assisted living, or healthcare experience strongly preferred
Excellent communication, relationship-building, and organizational skills
A professional, energetic, and positive approach
Strong follow-up skills and attention to detail
Ability to work independently while collaborating effectively with the facility leadership team
A genuine passion for helping residents and families navigate their healthcare options
