What You'll Be Doing

• Evaluate residents and develop individualized treatment plans focused on improving mobility, strength, balance, and functional independence.

• Deliver evidence-based physical therapy treatments that help residents reach meaningful milestones.

• Track progress, celebrate achievements, and modify treatment plans to maximize outcomes.

• Collaborate with nursing, therapy, and other interdisciplinary team members to provide exceptional resident care.

• Educate residents and families on mobility, safety, and strategies for maintaining independence.

• Maintain accurate and timely clinical documentation in accordance with regulatory and professional standards.

What We're Looking For

• Active Pennsylvania Physical Therapist (PT) license in good standing, or eligibility for PA licensure.

• Previous skilled nursing or long-term care experience is a plus, but not required!

• Familiarity with Net Health documentation software is preferred.

• Strong communication skills, a positive attitude, and a passion for helping others.

• A team player who enjoys collaborating, problem-solving, and making a difference.
