Position: Regional RN IV Hydration Nurse Service Area: Five Skilled Nursing Centers

Schedule: Full-time, five days per week

Work Arrangement: On-site travel and remote work Holiday

Position Summary

Harborview Health Systems is seeking an experienced Registered Nurse to serve as the Regional RN IV Hydration Nurse for five skilled nursing centers. This nurse will assess residents for changes in condition, review laboratory findings, identify potential hydration needs, and support the appropriate use of IV hydration services.

The Regional RN IV Hydration Nurse will collaborate closely with each facility’s Director of Nursing, management team, nurse practitioners, physicians, and other medical providers. This position requires regular travel between assigned centers, along with remote clinical follow-up, documentation review, education, and communication.

Essential Job Duties

· Visit each assigned center monthly to assess residents for laboratory needs, changes in condition, dehydration risks, and potential need for IV hydration.

· Review relevant clinical information, including laboratory results, diagnoses, medications, intake, output, weights, vital signs, nursing documentation, and recent changes in condition.

· Identify residents who may benefit from further medical evaluation or consideration for IV hydration.

· Communicate assessment findings and clinical concerns to the Director of Nursing, facility management, and appropriate medical provider.

· Complete the required UDA and other supporting clinical documentation accurately and timely.

· Maintain documentation that supports the resident’s condition, clinical need, provider notification, orders, treatment, response, and follow-up.

· Work with each Director of Nursing and management team to establish an effective process for identifying and following residents with hydration concerns.

· Collaborate with nurse practitioners, physicians, and other medical providers to encourage appropriate use and understanding of IV hydration services.

· Provide education to nursing staff and medical providers regarding signs and symptoms of dehydration, laboratory indicators, early intervention, IV hydration criteria, documentation requirements, and resident follow-up.

· Assist facility teams with reducing avoidable complications and hospital transfers related to dehydration when clinically appropriate.

· Follow up on residents receiving IV hydration to evaluate tolerance, response to treatment, laboratory results, and any additional clinical needs.

· Confirm that IV hydration services are provided according to practitioner orders, professional nursing standards, infection-control practices, and company policies.

· Maintain ongoing communication with facility leadership regarding residents at risk, identified opportunities, completed assessments, and needed follow-up.

· Participate in clinical meetings, audits, education sessions, and quality-improvement activities as requested.

· Provide remote support to assigned centers through medical-record review, telephone communication, virtual meetings, education, and clinical follow-up.

· Maintain organized records of facility visits, resident reviews, recommendations, interventions, outcomes, and educational activities.

· Protect resident privacy and confidentiality in accordance with HIPAA and Harborview Health Systems policies.

· Perform other related clinical duties as assigned.

Required Qualifications

· Current, unrestricted Registered Nurse license in the state or states of practice.

· Graduate of an accredited nursing program.

· IV therapy experience and demonstrated competency in IV assessment, initiation, maintenance, and complication prevention.

· Knowledge of hydration management, laboratory interpretation, infection prevention, and change-in-condition assessment.

· Skilled nursing, long-term care, acute care, infusion, or home-health experience preferred.

· Strong clinical assessment, documentation, education, communication, and organizational skills.

· Ability to work independently while maintaining effective collaboration with facility and regional leadership.

· Ability to manage responsibilities across five centers and complete required follow-up timely.

· Basic proficiency with electronic medical records, virtual meeting platforms, email, and remote-work technology.

· Valid driver’s license, reliable transportation, and ability to travel regularly between assigned centers.

Physical Requirements

· Ability to travel between multiple skilled nursing centers.

· Ability to stand, walk, bend, and perform nursing assessments and IV-related clinical duties.

· Ability to lift or assist with equipment consistent with the physical requirements of the position.

· Ability to use a computer for documentation, communication, education, and remote work.

Schedule and Travel Expectations

This is a full-time position scheduled five days per week, with major holidays off. The position includes regular on-site travel among five assigned Harborview Health Systems centers and remote work for clinical follow-up, documentation review, education, meetings, and communication.

Professional Expectations

The Regional RN IV Hydration Nurse is expected to demonstrate sound clinical judgment, professionalism, dependable follow-through, respectful communication, and a resident-centered approach. All IV hydration recommendations and treatments must be based on the resident’s assessed condition and completed under an authorized medical provider’s order.
