• Developing and maintaining relationships with clients, families, and healthcare providers to ensure seamless care coordination

• Identifying and addressing any gaps in care, ensuring continuity and quality of services

• Monitoring client progress and making adjustments to care plans as needed

Responsibilities

• Collaborate with interdisciplinary teams to develop and implement comprehensive care plans

• Communicate effectively with clients, families, and healthcare providers to ensure a seamless care experience

• Identify and address any concerns or issues that may arise during the care process

• Maintain accurate and up-to-date records of client care and progress

• Collaborate with the management team to develop and implement new processes and protocols

• Rotating on call

• Assist with recruiting and training
