Are you a Physical Therapist looking for more than just another therapy position?
We are a growing home health organization under new ownership and leadership, and we are looking for an exceptional Physical Therapist (PT) to join our team as we enter an exciting new chapter of growth.
This is a unique opportunity to get in on the ground floor of a growing organization and become an important part of the team we are building in Northern Colorado.
We're looking for someone who is passionate about patient care but also values independence, flexibility, communication, and being part of an organization where their ideas and contributions are truly valued.
If you're tired of feeling like just another employee in a large organization, we would love to talk with you.
As a Home Health Physical Therapist, you will provide skilled physical therapy services to patients in the comfort of their homes while helping them improve mobility, strength, safety, and independence.
Responsibilities include:
Complete comprehensive physical therapy evaluations in patients' homes
Develop individualized plans of care based on each patient's needs and goals
Provide skilled therapeutic interventions to improve mobility, strength, balance, endurance, and functional independence
Assess home safety and recommend appropriate equipment or modifications
Educate patients, families, and caregivers
Monitor patient progress and modify treatment plans as appropriate
Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary team
Complete timely and accurate clinical documentation
Help prevent falls, hospitalizations, and avoidable declines in function
Participate in care coordination and discharge planning
Home health gives you the ability to manage your day with greater independence while still having the support of a clinical team behind you.
We believe great clinicians do their best work when they're trusted to do their jobs.
You'll have the opportunity to work one-on-one with patients in their own environment, where seemingly small improvements can have a tremendous impact on their ability to remain safely and independently at home.
We're building a team—not filling slots.
We want clinicians who bring ideas to the table and want to be part of improving and growing an organization.
Current Colorado Physical Therapist license or eligibility for Colorado licensure
Graduate of an accredited Physical Therapy program
Current driver's license and reliable transportation
Excellent communication and organizational skills
Ability to work independently and manage a patient caseload
