Now Hiring:
We’re seeking a dedicated MDS Coordinator (RN or LPN) to lead and manage the RAI/MDS process and ensure accuracy, compliance, and quality-driven outcomes across the center.
Position Summary:
As MDS Coordinator, you’ll play a key leadership role in driving resident-centered care by coordinating timely and accurate assessments and care planning. You'll serve as a bridge between clinical teams and administration to ensure quality data and outstanding outcomes.
Key Responsibilities:
Serve as the facility's lead for all MDS/RAI processes and documentation
Initiate and lead Interdisciplinary Care Plan meetings
Coordinate and update resident care plans based on MDS triggers and assessments
Collaborate with nursing staff and other disciplines to gather accurate resident data
Ensure timely completion and transmission of MDS assessments per CMS and LTCI guidelines
Analyze Quality Measures/Indicators (QM/QI) from MDS reports and support performance improvement initiatives
Provide training and guidance to clinical team members as needed
Participate actively in quality improvement programs and audits
What We’re Looking For:
Must be a Licensed RN or LPN in the State of Georgia
Experience with MDS/RAI in a skilled nursing or long-term care setting strongly preferred
Strong understanding of CMS guidelines and compliance standards
Detail-oriented, organized, and collaborative
Excellent communication skills and the ability to lead interdisciplinary teams
