Now Hiring:
We’re seeking a dedicated MDS Coordinator (RN or LPN) to lead and manage the RAI/MDS process and ensure accuracy, compliance, and quality-driven outcomes across the center.

Position Summary:
As MDS Coordinator, you’ll play a key leadership role in driving resident-centered care by coordinating timely and accurate assessments and care planning. You'll serve as a bridge between clinical teams and administration to ensure quality data and outstanding outcomes.

Key Responsibilities:

  • Serve as the facility's lead for all MDS/RAI processes and documentation

  • Initiate and lead Interdisciplinary Care Plan meetings

  • Coordinate and update resident care plans based on MDS triggers and assessments

  • Collaborate with nursing staff and other disciplines to gather accurate resident data

  • Ensure timely completion and transmission of MDS assessments per CMS and LTCI guidelines

  • Analyze Quality Measures/Indicators (QM/QI) from MDS reports and support performance improvement initiatives

  • Provide training and guidance to clinical team members as needed

  • Participate actively in quality improvement programs and audits

What We’re Looking For:

  • Must be a Licensed RN or LPN in the State of Georgia

  • Experience with MDS/RAI in a skilled nursing or long-term care setting strongly preferred

  • Strong understanding of CMS guidelines and compliance standards

  • Detail-oriented, organized, and collaborative

  • Excellent communication skills and the ability to lead interdisciplinary teams
